Welcome to Sina Health. These Terms of Service (TOS) govern the use of our medical services, website, patient portal, SMS communications, and telehealth services. By accessing or using our services, you agree to abide by these terms. If you do not agree, please discontinue use of our services immediately.

 

1. Scope of Services

Sina Health is a multi-specialty medical practice providing in-person and telehealth services in Pennsylvania. Our services include primary care, specialty care, metabolic health optimization, psychiatry, and concierge medicine. These terms apply to all patients, website users, and anyone using our digital platforms or SMS messaging services.

 

2. No Medical Advice Disclaimer

  • The content on our website, blog, or social media is for informational purposes only and does not replace professional medical advice, diagnosis, or treatment.

  • Always seek medical advice from a licensed healthcare provider regarding any medical condition. Do not disregard medical advice or delay treatment based on information from our website.

  • If you have a medical emergency, call 911 immediately.

 

3. Patient Responsibilities

When receiving care from Sina Health, you agree to:

  • Provide accurate and up-to-date medical and insurance information.

  • Follow treatment plans and care instructions provided by our healthcare professionals.

  • Respect all appointment scheduling, cancellation, and payment policies.

  • Understand that failure to follow medical guidance may impact treatment outcomes.

 

4. Telehealth Services

By using our telehealth services, you acknowledge and agree that:

  • Telehealth visits involve electronic communication between you and a licensed provider.

  • Telehealth services may not be suitable for all medical conditions, and in some cases, an in-person visit may be required.

  • Sina Health follows HIPAA-compliant telehealth security measures, but no system is 100% secure. Patients should take precautions with their personal data.

 

5. SMS Messaging Service

By opting in to receive SMS messages from Sina Health and its affiliates ("we," "us," "our"), you agree to the following terms:

A. Consent to Receive Messages

  • By providing your phone number, you consent to receive appointment reminders, healthcare updates, and continuity-of-care messages.

  • Promotional messages are only sent if you opt in separately.

B. Message Frequency

  • You will receive messages as needed for appointment confirmations, reminders, and other medically relevant updates.

C. Message and Data Rates

  • Message and data rates may apply based on your mobile carrier’s terms.

D. Privacy Policy

  • Your information will be handled in accordance with our Privacy Policy.

E. Opt-Out Instructions

  • You can opt out at any time by replying "STOP" to any SMS message.

  • You may also contact us at (610) 904-8100 to request removal from SMS communications.

F. Liability

  • Sina Health is not responsible for any charges, errors, or delays in SMS delivery caused by your carrier or third-party service providers.

  • By opting in, you confirm that you own or are authorized to use the phone number provided and that you are at least 18 years old.

 

6. Insurance and Payment Policies

  • Sina Health accepts most major insurance plans. Patients are responsible for co-pays, deductibles, and any non-covered services.

  • Membership-based services (e.g., Direct Primary Care, Mental Health Direct, Metabolic Health Optimization, Concierge Senior Care) are not covered by insurance and are billed separately.

  • If you fail to pay for services, we may suspend non-emergency care until outstanding balances are resolved.

  • A no-show fee will apply for missed appointments based on our No-Show Policy Contract.

 

7. Prescription Policy

  • Prescription medications are provided based on medical necessity and provider discretion.

  • We do not prescribe controlled substances via telehealth except in accordance with Pennsylvania laws and DEA regulations.

  • Prescription refills require a follow-up consultation if deemed necessary by your provider.

 

8. Prohibited Conduct

When using Sina Health services, website, or digital platforms, you agree not to:

  • Use services for fraudulent, abusive, or illegal purposes.

  • Provide false or misleading medical information.

  • Share or misuse prescription medications.

  • Disrupt telehealth services or threaten staff members.

  • Attempt unauthorized access to our systems or patient records.

Violating these terms may result in termination of services and legal action if necessary.

 

9. Limitation of Liability

  • Sina Health is not liable for any damages arising from service delays, website downtime, telehealth limitations, or incorrect information provided by patients.

  • We do not guarantee specific medical outcomes or improvements in health conditions.

 

10. Modifications to Terms of Service

  • Sina Health reserves the right to update these terms at any time. Significant changes will be communicated via email, website updates, or in-office notices.

  • Continued use of services after changes means you accept the new terms.

 

11. Governing Law & Dispute Resolution

  • These terms are governed by the laws of Pennsylvania.

  • Any disputes should be resolved through mediation or arbitration before litigation.
